Asthma: When is it out of control?

 Our office treats many individuals with asthma. One of the most important things that I need to reinforce with my patients is that asthma is a livable condition, but, unless it is controlled properly, and patient understand the warning signs of “when the asthma is out of control”, the situation can turn deadly, very quickly. Fortunately, warning signs are usually present before this ever happens, and an asthmatic can have a normal and productive life.

Here are the warning signs I tell my patients about:

1.   Using your Proventyl inhaler more than usual. If you use it once a day, and now three times a day, the inflammation is the problem, and it’s the inflammation that needs to be addresses.

2.   Asthma, a cough or “tightness” that waked you up at night, especially between 3-5 AM is a warning sign.

3.   Shortness of breath that limits exercise more so than usual.

    This is not meant to be an inclusive list, but are 3 of the more common warning signs that I have seen  in 20 years of practice. Most importantly, talk to us about your asthma, and together we may be able to figure out your particular warning signs and show you ways to keep yourself in optimum health and condition.

Arthritis can have many causes

Although aesthetics are a large focus of my practice, keep in mind I have 20 years of experience in Internal Medicine and Pulmonary Medicine.  I am Board Certified in both Internal Medicine and Pulmonary Diseases, and I have  earned the reputation, substantiated by many of my associates and patients, as being one of the best in differential diagnosis and determining the cause of a person’s problems when many other doctors may have given up.

Arthritis can have many causes. It can be caused by degeneration, aging, being overweight or from an immune disorder, such as Rheumatoid Arthritis. Some of these disorders can be treated with an array of drugs, while others are more frustrating. Patients always ask me about Glucosamine and Chondroitin, or Knox Gelatin. They want to know if these can help. Often, you’ll see these combined with MSM, Magnesium, Calcium and anti-oxidants. My answer here is “I think they are over-hyped”.  The only thing that I have found to work, aside from things like Motrin, is exercise and omega-3 fish oils, which need to be taken in high enough doses in order to make a difference. My patients are usually recommended to take the fish oil from The Sear Zone, which sells pharmaceutical grade fish oil without any Mercury or PCB’s.  Usually 4 pills a day, but the dose may vary if the person is already on a platelet thinning drug, such as aspirin. Fish oils, are in many ways, the new Chicken Soup of medicine.

Your Internist can help tell you what type of arthritis you have, with a physical exam and some simple blood work and possibly an X-ray.  Even with the best treatment, sometimes arthritis can progress to the point of a joint replacement, but in general, we believe in an aggressive approach to treating the underlying disorder before this may become an issue.

The treatment of many different types of auto-immune arthritis has changed over many years, essentially with more aggressive and earlier intervention in the arthritis, especially Rheumatoid and Psoriatic arthritis. The use of “anti-TNF” drugs has revolutionized the treatment of these diseases, though these drugs carry risks, such as an increased risk of infections as well as possibly increasing the risk of lymphoma and leukemia. Some of these drugs have been withdrawn from the market due to the fact that they caused a re-activation of the JC virus in the brain and led to severe neurological effects.  This must be considered before starting any type of aggressive therapy.

What is Private practice? What is Concierge Medicine?

When I was growing up, I remember the doctor my mom used to see. He took so much time with her, listened to all of her complaints, and if he didn’t know the answer, he would research it and call her with the information. He made house calls when we were sick. He knew me, my family and even my dog!!

Something happened in Medicine- HMO’s destroyed the field. Plain and simple-some doctors made a lot of money and businessmen decided that they wanted it. So promised were made of cost cutting and better delivery of care. Neither of these has been shown to be true. Instead, doctors spent less and less time with patients, because they were forced by greedy administrators to see as many people a day as they could, because facilities were paid less and less to see a patient. The doctors couldn’t rebel, because if they organized against this madness, they were threatened with anti-trust law. So, the doctor I wanted to be was no the doctor I was allowed to be. Still, the public got lost in this fantasy of “House”…that somewhere a hospital has a team of “geniuses” they keep around to find out “what the problem is” and cost is never an issue. Tests are done, but nobody has to call for pre-approval. House never gets denied the chance to do the MRI he wanted. Mix in some drama, and you have a series. But, it’s just fantasy!

And then one morning, I said “why not”? Why not do it the way I think it should be done. I know more about medicine than the President, or a Senator, or a lawyer, or an MBA administrator. So, I decided to leave all the insurance plans and open up a “Private Practice”.  The model is simple.  I dedicate myself to each one of my patients and invest in the resources they need to keep them healthy and informed. They pay for my time and skill.  They know they will see me at their bedside if they are home sick and it will be ME calling them with the latest research on their disease. No 6 week waits, no getting blown off in the waiting room for 45 minutes, no rushed appointments!! 

One of my strengths has always been the difficult diagnosis. It’s a challenge. A test. It’s invigorating to have to think about what the problem might actually be. Medical detective, diagnostician, what ever you might want to call it. Is it a perfect model? No. Is it for everyone?  But it’s a choice. An option. …and that is what it should be for everyone!

Don’t Forget to Add Beauty to Your Wedding Checklist!

Of course every bride is beautiful! She has been planning this special day for months-the dress, cake, band, and the honeymoon just to name a few of the many details on her wedding “Checklist.” She feels ready and excited when the big day finally comes.

THEN the photos arrive! Just as soon as the pictures are in the brides hands…my phone starts to ring with new appointments! Unhappy brides and very often the mothers of the brides calling for help. Disappointed at the way their face looked “unrefeshed” in the wedding photos, wondering why they didn’t add seeing a dermatologist to their wedding checklist!

Why do Brides feel so scared to do “things” before the wedding? Aside from your hair and makeup, there is more generic fear about “having something done” as well as a fear that they will not look like themselves. Most of this fear comes from misinformation and not choosing the right Doctor to guide and council you. With the right Doctor and guidance the bride and Mother of the bride could feel relaxed that they will be just restoring their natural best! I always tell my patients that refreshing their look prior to their wedding or should be as important as the cake, photographer, dress and everything else!

So, I decided to write my first blog on Building Wedding beauty confidence. My goal is to improving appearance, reduce fear and most importantly, with a steady hand of reassurance help you achieve the look that will highlight your own best assets on that Special Day!

You see, the secret is not just looking great on that day, its looking great every day. So, when it comes to medical aesthetics, how should the bride prepare and what should she expect?

Lets take a quick look at how to schedule treatments and a few “do’s and don’ts” before the wedding.

1. Laser Hair Removal can take several months to work and you won’t typically see results till after the third session, though results may vary between people. Schedule up to 3-4 months prior to the wedding.

2. If you want fuller lips, remember it can take up to 2 weeks before everything ‘settles down” after the procedure. I recommend doing this 1 month before. We use either Restylane or Juvederm for the enhancement. With our approach, this is essentially painless. Schedule 1 month before wedding

3. Chemical peels and microdermabrasions: Your skin will look refreshed and radiant before the big day. We recommend doing this 1 month before your special day.

4. Botox takes 3-5 days to work. However, it case of a Botox complication, we strongly advise not doing it too close to the date. This is why we generally recommend Botox 6 weeks before and if you need a light touch up of more Botox; you can do it 3 weeks before, though individual cases vary. Schedule 1-2 months before wedding

5. Sun-Damage: A Photofacial is the catch phrase when it comes to sun damage. This is a very broadly used term for a certain technology that helps reduce redness and helps reduce the irregular look of photo damaged skin. Our office sees a lot of patients that have had injuries at other laser facilities. Given my research in lasers, I have been somewhat of an “unfortunate following” in this regard. In truth, this is the trickiest technology on the market, and when our office sees patients who have been injured with “IPL” or Intense Pulsed Light treatments, most of the time these injuries were avoidable. The results of a Photofacial are great, and we love doing these, but be sure that the person doing this procedure understands the technology and can deal with any adverse events that might occur. We will do Photofacials 6 weeks before the event. Schedule 2 months before wedding

Choose a doctor with years of experience or someone that has come highly recommended to you by a friend or family member. Don’t start with someone “new” that close to your wedding. Do ask questions. Don’t bargain hunt when it comes to your face. In many cases, you get what you pay for. Don’t’ chose someone who is $50 or $100 cheaper, but lacks the aesthetic experience or aesthetic eye, both of which are crucial when it come to your appearance.
